Works under the supervision of the Senior Director of Care Management to oversee the operation of the Care Management program in either Rochester or Buffalo. This includes the support, supervision, and hiring of staff, ensuring the delivering of quality services to all clients enrolled in the Care Management Program (and having a caseload), the adherence to all Health Home Guidelines put forth by the NYS Department of Health and meeting all of the reporting requirements to both the Health Home agencies and the NYS Department of Health.Provide support and supervision to staff regarding problem solving with other clients.Maintain accurate records and follow:up of on:going staff training as required by the agency, the Health Home agency and the NYS Department of pletes time cards and PTO requests, etc.Identify and enroll new clients and coordinate with the Senior Director to address any enrollment issues.Attend Risk Management meetings when necessary.Responsible for the overall client care as it relates to the Care Management program in that region.Ensures that each client receives comprehensive Care Management planning and assessments in a person:centered approach. Ensures that each plan of care is complete and that assessments are up:to:date and in accordance with the standard of practice as outlined by the NYS Department of Health.Ensures that annual appointments for physicals, eye exams and dental appointments are scheduled. Ensures that transportation is arranged for these appointments (when appropriate).Carries a caseload of 30:35 plete monthly and quarterly reporting as determined by DePaul, The Health Home, the NYS Department of Health and other County:specific reports.Respects and maintains client confidentiality and demonstrates a caring, position attitude towards all clients, staff, family members and service providers.Follows all safety rules and regulations for self, staff and clients.Provides the client with transportation if and when necessary.Performs any other duties necessary for the effective operation of the Care Management program.QMHS Standard. Minimum of a BachelorA?s Degree in Human Services with at least 1 full year of full:time experience in providing Care Management services to mental health consumers and at least 1 year of supervisory experience. Experience in working with Medicaid billable case management programs is preferred. ** In addition to the above criteria this position also requires a clean NYS driving record as outlined in DePaulA?s personnel policy.
